The purpose of the RN Care Manager is to case manage patients with Medicaid and those that are living in an Adult Care Home setting. They will be involved in assessing, planning, implementing, coordinating, monitoring and evaluating the care for this population.
RN (BSN preferred). Graduation from a state accredited school of professional nursing. Two years of experience in acute care or community based care preferred. Previous care management experience a plus. Computer proficiency needed.
Must possess a valid NC Driver’s License and a valid NC Nursing License.
This position offers a $2,500 bonus after the first 90 days (the end of a successful probationary period), $2,500 at the end of 6 months, and a $2,500 retention bonus after successfully completing one year of service.
